Beneficiary Gift
Beneficiary designation gifts are often considered an alternative form of bequest because they pass outside your will or living trust. Because they transfer directly to named beneficiaries, you do not need to modify your will or living trust to make these kinds of gifts.
How to leave a beneficiary designation:
- Request a “change of beneficiary” form from your plan administrator.
- List Bethesda Health Group Inc. as the beneficiary with the following information:
Bethesda Health Group Inc.
12101 Woodcrest Executive Drive, Suite 200
Creve Coeur, MO 63141
EIN#: 43-0666738
Gifts of the following assets may be transferred by beneficiary designation:
- Retirement assets include traditional IRAs, Roth IRAs, 401(k)s, and 403(b)s
- Life Insurance, Bank Accounts, Donor Advised Funds, Investment Accounts
