| Job Responsibilities: |
1.Initiates and oversees activity operations to ensure resident satisfaction and the proper completion of activities within a timely manner. Encourages resident participation in programs that meet the resident’s mental and physical needs.
2. Assists in the planning of effective individualized programming through written assessment, multi-disciplinary treatment participation and completion of required documentation according to established guidelines.
3. Assists in Providing goal-oriented and appropriate group and/or individual sessions designed to meet the needs of all residents and completes documentation on activity plan designed for each resident in a timely manner following established guidelines.
4.Prepares the display activity calendar to ensure that residents are participating in planned activities and aware of the activities that are available to them. Assists Activities Director in the development of programs that would best meet these individuals needs as well as continuing to meet the needs of other residents.
5. Demonstrates accountability for assessing, updating and maintaining resident skills through activities. Attends departmental meetings, in-service programs and sets annual professional goals.
6. Initiates activity charting and development of a activity program to meet resident’s physical and mental stimulation needs upon admission and continues to monitor residents weekly and completes charting as required. Assists in developing innovative new activities, programs and entertainment based upon the needs of all residents within the facility. Works with other departments to organize activities and assists in discussing new activity ideas with department managers. Assist in developing programs based upon resident needs.
7. Ensures that the necessary supplies and equipment is arranged for in a proper and timely manner prior to the activity. Works with team members to set-up for programs, gather residents and guests, initiate and encourage participation in activities, and clean up after each activity is completed.
8. Welcomes and prepares for new residents, visits residents on special occasions, writes and creates written and visual displays for resident appreciation. Monitors activity in the activity center and keeps work area clean and free of anything that could be harmful to residents. Assists in decorating or set-up for special events.
9. Seeks guidance and direction as necessary for the successful completion of job duties, utilizes the appropriate chain of command and keeps immediate supervisor aware of any expected changes and potential problem areas in a timely manner.
10. Assists in applying standards of performance improvement to exceed the needs of the residents and improve services. Participates in departmental performance improvement activities.
11. Performs other duties as assigned.

2. Documents appropriately into patient record charting based on skill level and follows established standards and guidelines of the nursing department.
3.Upon admission, accurately completes personal belonging sheet, obtains and records height, weight and vital signs, orientates new resident to the facility and unit, ensures that resident receives a arm band and provides resident with a labeled admission kit.
4. Successfully protects resident privacy in all aspects of care. Ensures that Residents Rights are protected and that resident concerns are addressed in a professional and timely manner. Treats all residents kindly and with respect at all times.
5. Answers call lights promptly and responds to all resident needs. Completes rounds as assigned and throughout course of duty. Turns and repositions residents every 2 hours. Repositions seated residents every hour.
6. Takes vital signs and weights accurately and as instructed by Charge Nurse and reports to charge nurse promptly and accurately. Ensures that proper slings, splints, braces, positions devices, specialty care items, etc. are applied according to physician or therapy orders.
7. Accurately measures intake and output on the I & O sheets and relay information to Charge Nurse at shift
change. Performs procedures and treatments as instructed by Charge Nurse (i.e.: Sitz bath, moist packs,
isolation techniques, etc)
8. Maintains appropriate supplies and equipment on the unit and alerts charge nurse or purchasing clerk of additional needs. Ensures that charge items are entered according to departmental procedure.
9. Successfully completes job duties by attending continuing education classes and in-services, accepting reassignments to other areas, taking on increased responsibility and keeping Charge Nurse informed of pertinent information that may require follow-up.
10. Demonstrates unit-defined competencies as required.
11. Interacts and maintains effective working relationships with other team members, charge nurse and department manager. Maintains discretion in handling confidential information. Gives report to charge nurse at the end of each shift and completes walking rounds with the oncoming or leaving shift.

1. Properly sets up dining room tables for the next meal, stocks beverage area and prepares necessary items for the following shift including silverware or folded napkins as assigned.
2. Responsible for serving residents in the dining room including beverages, meals, condiments and any special need that the residents may need or request per diet. Employee remains in the dining room throughout the meal to refill items, and meet special requests.
3. Cleans dining room and work areas as assigned, follows department procedures and utilizes correct cleaning products 100% of the time. Follows established department

| Job Responsibilities: |
1. Within the tour of duty accurately assesses residents’ physical and psychosocial status, educational and environmental needs of assigned residents. Identifies needs for other services/disciplines for residents and intervenes appropriately to secure these services. Documents admissions and completes ongoing nursing assessments on assigned residents.
2. Develops measurable goals, admission and discharge plans with physicians, residents, families and multidisciplinary team members. Document and communicates plans appropriately and professionally.
3. From continual assessment, reviews and revises the care plan needs, problems and goal changes. Communicates
changes with Patient Care Coordinator. Identifies necessary and appropriate nursing interventions pertinent to the plan of care. Implements and documents all resident/family teaching relevant to health care needs and ensures understanding of the teaching.
4. Demonstrates clinical competence to assess the physical, spiritual, psychological and emotional needs of each resident. Intervenes appropriately requesting physician or other department intervention as appropriate. Ensures that documentation is completed in a prompt and professional manner. Ensures that all information is recorded and correct prior to contacting the resident’s physician for assistance or advise.
5. Collaborates and communicates, with tact and sensitivity, with other team members and departments to coordinate a multidisciplinary approach to resident care. Identifies and interprets the psychological reaction of the residents and takes appropriate action within the time frame necessary. Documents nursing interventions and resident response to care accurately, timely and completely.
6. Administers resident treatments and procedures in accordance with physician orders and nursing policies and
procedures. Ensures that all treatments are signed for and completed in the appropriate allotted time frame.
7. Administers medication and IV fluids skillfully, safely and timely. Accurately administers medications and signs for medication according to policy and within the allotted time frame 100% of the time.
8. Handles crisis situations in a prompt, precise and professional manner. Actively participates in the direct delivery of patient are. Completes rounds to ensure that residents are receiving the appropriate nursing care in an appropriate time frame. Ensures that a safe, comfortable, clean environment is maintained.
9. Monitors resident physical and chemical restraints to ensure resident safety. Works with ADON to reduce or eliminate restrains when feasible. Trains assigned staff on the use of restraints, treatments, positioning
devices, etc. to ensure a complete understanding of their function and provide resident safety.
10. Organizes information and communicates with physicians and supervisor regarding relevant patient care
issues. Coordinates activities on the assigned unit to ensure appropriate functioning of the unit and
continued quality care. Transcribes/verifies physician orders, coordinates testing, treatments and
procedures with other departments. Promptly co-signs orders and clarifies as necessary.
11. Empowers staff by promoting responsibility, accountability, and autonomy to enhance patient care.
Completes CNA assignments and follows up on responsibilities to ensure residents satisfaction. Ensures
standards of patient care are utilized and delegates responsibilities as defined by policy or necessary
to ensure appropriate resident care. Monitors and assist assigned staff as necessary to ensure completion
of all tasks properly and in a timely manner.
12. Demonstrates flexibility in responsibilities and professional competency at all times. Participates in
in servicing, continuing education and other activities to keep professionally current. Maintains working

1. Records absenteeism and tardiness on the daily assignment sheet. Assigns staff according to policies and works to ensure equal distribution of assigned duties. Supports team work and works to encourages staff to provide resident care properly and in an acceptable time frame.
2. Completes rounds to assess residents and resident care. Identifies concerns and need. Reports concerns and needs to appropriate staff members in a professional and timely manner. Receives report from assigned charge nurses in a timely manner and completes a shift report for the Director of Nursing.
3. Communicates with Director of Nursing areas of concern of any events requiring administrative attention in a timely manner. Appropriately manages staff and disciplines staff members as necessary. Empower staff members by promoting responsibility, accountability, and autonomy to enhance resident care. Ensures standards of patient care are utilized and delegates responsibilities as defined by policy or necessary to ensure appropriate resident care.
4. Develops measurable staff goals and encourages staff to maintain professionalism within the work place at all times. Serves as a resource for other nursing personnel and seeks assistance or guidance in areas of uncertainty. Directs and manages direct patient care by using sound nursing judgment at all times. Document and communicates plans appropriately and professionally
5. From resident assessment, reviews and revises the care plan needs, problems and goal changes with the Charge Nurses. Identifies necessary and appropriate nursing interventions pertinent to the plan of care. Implements and documents all resident/family teaching relevant to health care needs and ensures understanding of the teaching.
6. Demonstrates clinical competence to assess the physical, spiritual, psychological and emotional needs of each resident. Works with Charge Nurses to maintain their clinical competencies and intervenes appropriately requesting physician or other department intervention as appropriate.
7. Collaborates and communicates, with tact and sensitivity, with other team members and departments to coordinate a multidisciplinary approach to resident care. Identifies and interprets the psychological reaction of the residents and takes appropriate action within the time frame necessary.
8. Handles crisis situations in a prompt, precise and professional manner. Actively participates in the direct delivery of patient care. Completes rounds to ensure that residents are receiving the appropriate nursing care in an appropriate time frame. Ensures that a safe, comfortable, clean environment is maintained.
9. Organizes information and communicate with physicians and staff members regarding relevant patient care issues. Coordinates activities and assignments throughout the facility to ensure appropriate functioning of each unit and continued quality care. Analyzes problems, offers recommendations, and initiates problem resolution in a professional and timely manner. Investigates employee concerns or incidents and takes the appropriate action. Communicates to the Director of Nursing action taken and seeks advice in necessary situations.
10. Assigns employees in the most efficient and effective manner to meet the needs of the department, residents and health system. Promotes teamwork with employees and delegates responsibilities within the scope of the employees abilities.
11. When appropriate, assists employees in resolving unsatisfactory performance and behavior issues in a positive and constructive manner, ensuring appropriate behaviors are explained and the expected standard of performance is communicated. Conducts performance evaluations of employees according to the established procedures and within established Human Resources time frame.
12.Demonstrates flexibility in responsibilities and professional competency at all times. Participates in
in servicing, continuing education and other activities to keep professionally current. Maintains working
knowledge of emergency procedures and demonstrates awareness of safety issues relevant to patient and
employee safety. Administers yearly flu vaccine program to residents and employees.
13. Communicates effectively with residents, visitors, co-workers, physician and other team members using professionalism. Collaborates with physicians and other departments for problem solving and communication of resident related issues in coordinating the resident’s plan of care. Accepts or denies new resident admissions. Implements PPD program for residents.
14. Interviews applicants for CNA, CMT, and Nurse Assistant positions. Greet new resident admits.
15. Completes monthly CQI, daily nursing labor forms for Administration, and monitors pharmacy recommendations monthly. Attend Team Leader meetings twice per month and CNA meetings monthly.
